Guest Pass Application

• • The application must be completed and turned in to the Student Store by the WEDNESAY before the dance.
    
    

  • The guest’s Student ID is required at the door and a photocopy of the Student ID must also be attached to this form. If the guest has graduated, a driver’s license or Official ID is required.
    
    

  • The guest pass form is to be returned ONLY to the Student Store. DO NOT turn it in to anyone else.

    • Middle School students and students over 20 may not receive a guest pass.
      
      

  • The guest pass requires both a signature and business card from the guest’s school. We will not process without both
